We make it easy to navigate your patient portal. Click on the following links to sign in, reset your password, or reset credentials.

  • Sign-in: Enter your username and password for access to the Patient Portal
    • You can't create a new account from here. You must already know your username and password.
  • Reset password: Update your password if you can’t remember it
  • Reset credentials: Contact us to reset on the logon ID
  • Create an account: Sign up for an account
    • You can't create an account unless we have your e-mail on file first. Please contact a receptionist with your e-mail if we don't already have it.

Digital Forms

Your health information/medical records are available on the patient portal for the timeframe of 04/01/2019 to present. To request records prior to that, please fill out the form below.

  • We will process your request within 30 days.

Release of Information Request Form

To revoke or grant access to a proxy on our patient portal, fill out one of the forms below.

**If you have an existing Patient Portal, please share your access, rather than requesting proxy access. In order to share access, the person you're sharing with needs to have been a patient at FMDH in the past and have an email on file with us.

  • Please allow 7 business days to process patient proxy access requests.
